Online Forms

Below are links to the online forms for this year's Festivals and Workshops. These forms will help streamline administration of the Festivals and eliminate errors. If you already have the information in an electronic document, you should be able to cut and paste the information into these forms. Unfortunately, you do have to be online to submit the forms, so please look at them before they are due and assemble the requested information. When you have entered the information and clicked on the "Submit" button, you should be sent a copy of the E-mail you have sent Georgia with your information. If you receive an error, go back and check to make sure that you have submitted any required information. This required information is usually a valid E-mail and sometimes a Festival name. Arrival Departure Form

Please use this form for all Festivals and Workshops

The fields for Festival and E-mail must be completed.

Please give as detailed information as possible as ground transport arrangements and housing connections are made using this information.

Registration Form

Please use this form for all Festivals and Workshops with the exception of AMIS International Honor Band and Choir Festival, Workshop for Teachers of Students of IB Music and the Solo and Ensemble Festival.

For Festivals ou will need a copy of the AMIS Acceptance Agreement signed by parents and student.

Information needed to complete the online form:

School name; School Address; School Telephone: School Fax; Music Teacher(s) Names; E-mail Contact; Home Telphone Number

For each student accepting/registering;

Name; Sex; Nationality;Part/Instrument; Grade; Telephone Number; Accepts

Housing Form

Please use this form for all Festivals and Workshops with the exception of AMIS International Honor Band and Choir Festival and the Solo and Ensemble Festival.

The fields for Festival and E-mail must be completed.

Online Track List and Entries Form

Please use this form for all AMIS International Honor Jazz BAnd, AMIS International Honor Band and Choir Festival, AMIS European and Asiain Middle School Honor Band and Junior and Senior Honor Orchstra Festival.

The fields for Festival, School, Directos and E-mail must be completed. You only need to put the school address, home and school telephones, and fax on your first entry.

Please complete one form for each instrument or voice auditioning. Please, do not put more athan one instrument or voice on a cd/tape/minidisc.

Please list the performers in the same order as they appear on your cd/tape.minidisc.

Information required:

• Performer name (As it will appear in the program...then you won't need to fix it later)
• Grade
• Performer number
• What track number their audition begins

AMIS International Honor Band and Choir Festival

Honor Band and Choir Festival Acceptance Form (Twenty students)

Information needed

• Director and school information. E-mail address is required.
• You will need a copy of the AMIS Acceptance Agreement signed by parents and student.

Honor Band and Choir Housing Form

Information needed

• Director and school information. E-mail address is required.
• Chaperones, director and chaperone housing, allergies and information, director's dinner, and student room mate requests.

Director's Expertise Form

Information needed:

• Directors names and one contact E-mail.
• Director's special areas of expertise.
• Accompaniment help
• Rehearsal CD help
• Would you like to be considered for the audition panel?
• Would you like to offer a workshop at a future Festival?
  

AMIS Solo and Ensemble Festival

Intention Form

Information needed:

• Director and School information. E-mail address is required.
• Approximate number of entries anticipated.
• Request for hotel information.
• Prepayment plan.

Participants List

Information needed:

• Director and School Information. E-mail address is required.
• Full name of every student participating.

Summary of Entries

Information needed:

• Director and School Information. E-mail address is required.
• Entry Number, Jr/Sr, Ensemble Leader or Soloist, Accompanist, Number in Ensemble.(Up to 50 entries, re submit form if necessary.)
• List of students in more than one event: Student Name and Event Numbers - Ensemble events with leader's name as in summary above.
