Student Festival Agreement

PROCEDURE CHANGE: For Festivals you will need a copy of the Student Festival Acceptance Agreement signed by parents and student. Please print the information legibly, make a copy for your records and bring this form to the Festival. Please go to the relevant Festival page (menu on the right) to download the Agreement form for the particular Festival a student is attending.

Information needed to complete the online form:

School name; School Address; School Telephone: School Fax; Music Teacher(s) Names; E-mail Contact; Home Telephone Number

For each student accepting/registering;

Name; Gender; Nationality;Part/Instrument; Grade; Telephone Number; Accepts.
